Jr. Testing  -  9:00am
Jr. Adult & Adult Testing  -  10:30am

Please arrive 10 minutes early ready to go.  Once the test begins there will be no late arrivals allowed.

Cost is $45.00   -   Submit this completed form and test fee (check or cash) by the Thursday before.
